How do I refer a patient to Prime Vitality Health?
Referring a patient is straightforward. Download our Patient Referral Form, complete it with the patient's demographic, clinical, and insurance information, then fax it to our secure line or upload it through our secure portal. Our clinical team — led by Dr. Shiv Goel — reviews every referral within the same business day and contacts the patient or facility to schedule the first visit. What forms do I need to complete for a referral?
For most referrals, you'll need to complete the Patient Referral Form. If your patient has a wound, also include the Wound Care Referral Form. The patient will need to sign the Consent to Treat and HIPAA Authorization before services begin. For patients receiving telehealth or remote monitoring through TimeVitality.ai, the Telehealth Consent is also required. All forms are available in the download center above.
How quickly are referrals processed?
We offer same-day referral processing. When a completed referral is received during business hours, our clinical team reviews it and coordinates the patient's first visit — typically within the same business day. For urgent wound care or time-sensitive post-discharge follow-up, please call us directly at 210-251-4911 so we can prioritize the case accordingly.
Do you accept referrals for wound care specifically?
Yes. Advanced wound care is one of our core specialties. We accept referrals for chronic wounds, pressure ulcers, diabetic foot wounds, venous and arterial ulcers, surgical wounds, and more. Please use the Wound Care Referral Form so our team can prepare the appropriate supplies and treatment plan before the first visit.
Is there a contract required to partner with your practice?
No long-term contract is required to begin referring patients. You can download the Facility Partnership Agreement to review our standard partnership terms, but you are not obligated to sign a contract before sending your first referral. Many of our facility partners simply begin referring patients and formalize the relationship after experiencing our quality of care. Contact us to discuss your facility's specific needs.
What insurance does Prime Vitality Health accept for referred patients?
We are in-network with Medicare and most major commercial insurance plans. Our services are fully billable through Medicare Part B for eligible beneficiaries, including visiting physician visits, wound care services, remote patient monitoring through TimeVitality.ai, and concierge medicine at home. Can I refer patients from multiple facilities?
Absolutely. We work with discharge planners, social workers, and referring physicians across hospitals, SNFs, ALFs, rehabilitation centers, and hospice programs throughout San Antonio. You can refer patients from as many facilities as you work with. For high-volume referrers or multi-facility partnerships, we recommend reaching out to discuss a formal Facility Partnership Agreement and dedicated clinical liaison support.
